Robert Kaplan, M.D.
Doctor Robert Kaplan has been making house calls for almost 30 years. He received his Medical degree from Northwestern University Medical School in 1979 and completed his residencies at the University of Illinois Medical Center and the Veterans Administration Hospital. Additionally, he has extensive research experience as both a Principle-Investigator and Sub-Investigator in various medical research studies.
John Mason, PA-C
John Mason, PA-C, is a graduate of Alderson Broaddus University with a Master's Degree in Physician Assistant Studies. He has over 20 years of emergency medicine experience and is now focusing on home healthcare for disabled and home bound patients. John is married and has an 8 year old daughter. He enjoys golf, physical fitness and spending time with his family.
April Ledell-Hughes, DPM
April Leddell-Hughes is married with 2 kids. She was born and raised in Chicago and graduated college from the University of Illinois Urbana Champaign in 1999. She went on to graduated from Dr. William M. Scholl College of Podiatric Medicine in 2003 and has been practicing podiatric medicine ever since. Throughout the past 13 years she has practiced in different settings such as hospitals, nursing homes, offices, and home care visits.
Annell Tucker, DPM
Annell Tucker is a Podiatrist and has been providing podiatric home care for over 3 years. She completed her undergraduate at Oakwood University in Huntsville, AL. She is a graduate of Scholl College of Podiatry and completed her residency training at Central Alabama Veterans Health Care System.
Zachary Jagmin, DPM
Dr. Jagmin was born and raised in Chicago’s south suburbs. Dr. Jagmin completed his undergraduate degree in biology at the University of Illinois, Champaign-Urbana in 2007 and his doctorate in podiatry at Temple University (Philadelphia, PA) in 2011. Dr. Jagmin completed his residency training at Massachusetts General Hospital (Boston, MA) in 2014. Dr. Jagmin was eager to return to the Midwest following completion of residency, and he has a special interest in providing home health care podiatry services. Dr. Jagmin is a board certified podiatrist by the American Board of Podiatric Medicine. Dr. Jagmin’s hobbies include music and reading.
Cynthia Carlson, RD
Cynthia Carlson; RD, LDN is a Registered Dietitian with The Academy Of Nutrition & Dietetics. Cynthia graduated with a Bachelors of Science from Northern Illinois University. Cynthia has been in practice over 20 years. Specialties include but are not limited to Diabetes Management & Chronic Kidney Disease. As well as Heart Health, Weight Management, Food Sensitivities, Gastrointestinal Disorders and Wellness. Through my love of food and passion for helping others I empowers others to achieve their nutrition and lifestyle goals while living the best life possible.
Kila D Nsamenang
Kila joined In-Home Physicians in November 2019. Prior to that, she has been a physician assistant in family medicine since August of 2009. Her mission is to treat people with care and compassion and understand the root cause of their problems, how it affects their health and provide healing or comfort.
Stacy graduated from UIC and Purdue Global University and has been a nurse for over 25 years. Through her experiences practicing as an RN and an Advanced Practice Nurse in home care setting, she has seen how patient miseducation, patient miscommunication, and lack of holistic care can cause disease exacerbation and even land patients into the hospital. Her belief is that education is one of the most proactive measures a nurse can take to prevent acute illness. Viewing each patient as a person and holistically is key in achieving better outcomes. She loves science and is fascinated by the beautiful complexity of the human body. In her spare time she enjoys gardening, reading, hiking and cycling.
Jovan Butler MSN, FNP-BC
With a Bachelors of Science and a Masters of Science in Nursing from Olivet Nazarene University (May 2011 and May 2018), Jovan is a Board Certified Family Nurse Practitioner with experience in Adult and Family Medicine. She holds both a master’s and bachelor’s degree in nursing from Olivet Nazarene University, Bourbonnais Illinois. Before attending graduate school at Olivet, Jovan practiced in the Emergency and Intensive care areas. She Believes in developing strong relationships, in order to provide personalized care, tailored to meet each patients specific needs. She joined In-Home Physicians in 2020.
