We are so excited to announce that In-Home Physicians has been selected to join The Illinois House Call Project! We are honored to have been chosen and look forward to working with the Home Centered Care Institute (HCCI) in such an important effort.
As a part of the HCCI Practice Excellence Partner (PEP) Network, we will be among the leading house call programs recognized nationwide as premier HBPC providers and serve a key role in Illinois as well as the national movement to improve patients’ access to high-quality home-based primary care.
The goals of the Illinois House Call Project include increasing access to home-based primary care for 3,000 new Home Based Primary Care patients, elevating awareness, and promoting the value of HBPC for the healthcare community, the policymakers, and the public.
